Patient characteristics associated with unmet needs post-hospital discharge

Join Stanford Medicine Health Care's Clinical Advice Services for a research webinar where we will present findings from a retrospective analysis exploring health determinants associated with one or more unmet needs following hospital discharge, as well as the link between these unmet needs and 30-day hospital readmissions.

This is a virtual class via Zoom.

**This will be a live webinar and you will need a computer with internet access to view the meeting. Instructions on how to login will be emailed upon registration.

1. Identify the top three unmet needs of the Stanford Health Care patient population post-hospitalization.
2. List the potential implications for patients and organizations if these needs remain unmet and interventions to address them.
3. Discuss the impact of implicit bias on the post-hospital experience.
